I hereby agree:

  1. that Regulation 4(1) of the Working Time Regulations 1998 will not apply in relation to my working hours [providing that (insert any specific conditions that the employee requires which amount to provisos of his/her agreement to disapply Regulation 4(1))].
  2. to work in excess of 48 hours per week on average if reasonably necessary pursuant to the obligations under my contract of employment; and
  3. to provide the Company with the above notice in writing of my intention to terminate this agreement.
