I refer to our meeting at your home on [date].

I was very sorry to hear that your condition has not improved and that it is unlikely that you will be able to resume working.

As we discussed, there is little we can do to assist your return to work and our medical adviser has reported that you are not likely to be well enough to return to to your current job for some time, if at all. We have tried to find some alternative suitable work for you but unfortunately there is nothing we can offer you because [explain reasons].

I regret that I have no alternative other than to give you notice to terminate your employment with the Company with effect from [date].

You are entitled to full pay for the period of your notice plus accrued holiday pay. I shall arrange for these sums to be paid to you, and for your P45 to be sent to you as soon as possible.
